- Screen for prior contrast reaction → follow slow or rapid premedication guidelines. Be ready with ACLS.
- Check eGFR (< 30 or rapidly declining → consider non‑contrast)
- If aspiration risk → use barium or avoid PO contrast
- If perforation or SBO concern → use water‑soluble PO contrast (gastrografin). Avoid barium.
